Appointment Scheduling

If you are a new patient and wish to make an appointment to see Dr. Donthineni, please ensure that the following steps are taken so that your appointment can be scheduled as soon as possible.

  1. If you have been seen previously by another physician for the same condition, please contact that medical office and have them send us any relevant clinical, diagnostic, and operative records via fax at 510-594-2275.
    • If you are being referred by another doctor, please have that office send a referral that indicates that a consult is being requested and the reason for the referral.
  2. Please obtain copies of any imaging such as X-rays, MRIs, or CT scans and bring these to your appointment. Please note that you should bring both the printed reports and the actual films (or CD) for each relevant diagnostic exam to your appointment.
  3. Contact our office at 510-594-9411 and schedule an appointment. Remember to have your medical insurance information available, as we will take this information over the phone at the time of scheduling your appointment.
  4. Once your appointment has been scheduled you should download the registration forms from the ‘Forms’ page of our website, fill them completely, and bring them with you to your appointment. The three documents that you should download are: Patient Info, History Form, and HIPAA form. If you are unable to download and print these forms you may supply us with your email, fax, or physical address, and we will be happy to send the forms to you.
  5. After checking your eligibility and benefits, if it is determined that your insurance requires authorization before you may be seen in our office, we will contact you and you may have to reschedule your appointment until after an authorization has been obtained.
